The Group Note wizard provides a centralized, guided experience for documenting group therapy sessions in TherapyNotes®. Instead of opening each patient’s chart individually, clinicians can complete all required group session documentation from a single workflow.
This workflow supports the DAP (Data, Assessment, Plan) format and helps reduce repetitive data entry by allowing shared group information to flow into individual patient notes.
Access the Group Note Workflow
You can open the Group Note workflow for a group therapy appointment by visiting:
- The To-Do page through a consolidated To-Do item or To-Do > Notes tab
- Scheduling > Click the appointment > Notes tab
Group Note Workflow Overview
The workflow is divided into three guided steps:
- Session Details, Attendance & Note Status
- Group Information
- Individual Patient Notes
Step 1: Session Details, Attendance & Note Status
In this step, review and update the group session details and participant attendance information.
For each participant, you can:
- Mark attendance status
- Adjust individual start and end times
- View the current note status
If a group appointment is canceled, all participants are automatically marked as missed. If the appointment is reinstated, attendance statuses return to their previous state.
Step 2: Group Information
The Group Information step is where you enter shared DAP content for the session.
This section allows you to document common information once and automatically carry it forward into each participant’s individual note.
Examples of shared information may include:
- Group topic or focus
- Shared interventions
- General group observations
- Planned next steps
Complete the required Data and Plan fields to proceed to Step 3.
Step 3: Individual Patient Notes
In this step, you will complete documentation for each participant individually.
Depending on attendance status, TherapyNotes will generate:
- A DAP Progress Note for attended participants
- A Missed Appointment Note for participants marked as missed
As you complete notes, the workflow automatically guides you to the next participant after:
- Selecting Save & Next, or
- Saving the note as a draft
This helps streamline documentation for the full group session.
Treatment Plan Requirements
If your practice requires treatment plans and a participant does not have a valid treatment plan on file, TherapyNotes will display a notification during the workflow. You can proceed to create the required treatment plan before completing the participant’s progress note.
Reviewing and Navigating Notes
As notes are completed, clinicians can:
- Open completed notes from the note status links in Step 1
- Use navigation buttons at the bottom of the workflow or top-left side navigation to move between steps and participants
- View completed notes by visiting:
- Scheduling > Click the appointment > Notes tab
- Patients > Patient name > Documents tab
When viewing the Group Progress note from the Documents tab, notes associated to Group Therapy session will include a link in the top-right corner labeled View All Notes for Group Session to return to the workflow. The link will only appear if Step 1 of the workflow (Session Details) has been saved.
When completing Step 2 (Group Information) and Step 3 (Individual Patient Notes), a top-left step navigator will display to help track progress, see completion status, and quickly jump between patients.
