Including a diagnosis on a prescription improves patient safety by helping pharmacists verify that the medication matches the intended condition, reducing the risk of errors. It enhances communication between healthcare providers, supports continuity of care, and can improve patient understanding and adherence. In many cases, it also meets legal or insurance requirements, particularly for prior authorizations or audits. Additionally, linking prescriptions to diagnoses supports data tracking, quality improvement, and public health monitoring.
Entering a Diagnosis
Add New Diagnosis
- Navigate to Patients > Patient Name > eRx tab
- In the Diagnoses / Problems section, select the green Add Diagnosis button.
- Enter the diagnosis along with the preferred ICD code, such as ICD-9 (Complete), ICD-10 (Complete) or ICD-10 (Billable) and select from results displayed.
- Click Ok to add the diagnosis to the patient’s chart.
Report No Diagnosis
If the patient has no known diagnosis, select NKD.
Make Inactive, Resolve or Delete a Diagnosis or Problem
Make Inactive
An inactive diagnosis indicates a medical condition that a patient has had in the past but is no longer active, relevant, or currently being treated.
To mark a diagnosis as inactive, hover over the drop-down icon next to the diagnosis entry and select to Make Inactive.
Mark Resolved
A Resolved diagnosis indicates a medical condition that was previously active but has been successfully treated or has gone away on its own, and no longer requires ongoing monitoring, treatment, or intervention.
To mark a diagnosis or problem as resolved, hover over the checkmark icon next to the diagnosis entry and select to Resolve.
Delete a Diagnosis / Problem
Deleting a diagnosis or problem is a permanent action and cannot be restored to a patient’s chart.
- Hover over trash can icon next to the diagnosis entry you want to remove.
- Click to Delete.
- A confirmation message will appear asking if you’re sure you want to delete the diagnosis.
- Click Confirm Delete to remove the diagnosis from the patient’s chart permanently.
