The Client Feedback Form is a brief survey designed to measure your satisfaction with your most recent session. It helps your therapist and their practice better understand your experience and identify opportunities to improve your care. The form takes less than three minutes to complete.
Navigating to the Client Feedback Form on TherapyPortal
To protect your privacy, you must be signed in to your TherapyPortal account to complete any forms sent by your clinician or practice.
When a new form is available, you’ll receive an email notification letting you know it’s ready to complete. If you receive appointment reminders by email, those messages will also include a note about any outstanding forms. Both types of emails include a link to the TherapyPortal sign-in page.
Once signed in, any available forms, such as the Client Feedback Form, will appear on your home page. You can also click Documents at the top of the screen to view your full list of forms. Select the Client Feedback Form to get started.
Completing the Client Feedback Form in TherapyPortal
From the TherapyPortal home page, click Needs Your Attention – 1 document to review, or navigate to the Documents tab. Select Client Feedback Form. Then, click Answer Questions to begin.
You’ll be taken to an overview page with details about the Client Feedback Form and instructions for completing it. For each statement, select the option that best reflects your experience during your most recent session. When you're ready to begin, click I'm Ready. Let's Begin!
The form includes six questions. Select your response for each one, then click Continue to proceed. A green progress bar at the top of the screen will show your progress as you complete the form.
To return to a previous question, click Back. To exit the form and return later, click Cancel.
After you complete all questions, a confirmation message will appear:
"Your responses have been submitted. Thank you!"
