The Client Feedback Form helps clinicians understand how clients perceive the quality of their therapy sessions. It can be administered by the clinician or completed independently by the client through the TherapyPortal. The form is available in both English and Spanish. Results can be viewed in the client’s chart, printed, or downloaded as a PDF.
Clients respond to six questions, rating their experience on a scale from Strongly Agree to Strongly Disagree, and select the option that best reflects their feelings.
Send the Client Feedback Form to a Client
You can send the Client Feedback Form to a client in two ways: from the Outcome Measures tab in the Library or from the client’s Portal tab. Additionally, you can set the Client Feedback Form to be Always Available on the TherapyPortal for your clients to complete at their convenience.
- Click Library > Outcome Measures tab.
- Click on Client Feedback Form from the Outcome Measure list.
- Click Share on Portal.
- Type in your client’s name.
- Enter any instructions you’d like your client to consider when completing the form.
- Click Send Document Request.
You can also send the Client Feedback Form in the patient view:
- Click Patients > Patient Name > Portal tab > Share Documents button.
- The Share Documents on Portal dialog will appear. In the dropdown menu, select Outcome Measures.
- Type in or select Client Feedback Form to send to your client.
- Enter any instructions you’d like your client to consider when completing the form.
- Click Send Document Request.
Reviewing a Completed Client Feedback Form from TherapyPortal
You can view a completed Client Feedback Form from the client’s Documents tab or your To-Do List under Process Portal Responses. Click Client Feedback Form to review and process the submission.
Administer the Client Feedback Form as a Clinician
Clinicians can also administer the Client Feedback Form during a session in one of two ways: from the Outcome Measures tab in the Library or from the client’s Documents tab.
- Click Library > Outcome Measures Tab
- Click on Client Feedback Form in the Library.
- Click Administer Now.
- Type in the full or partial name to look up a patient.
- Click Start to begin.
You can also administer the Client Feedback Form from the patient’s Documents tab.
- Click Patients > Select Patient > Documents Tab
- On the upper right-hand side, select the Outcome Measure dropdown and select Client Feedback Form
Reviewing the Clinician-Administered Client Feedback Form
- The completed Client Feedback Form will be available in the patient’s Documents tab.
- The Status column will display Completed when it is clinician-administered.
Processing the Form
After reviewing the form, click Mark as Processed at the bottom of the screen to update the status. All sent and completed questionnaires remain available in the client’s Documents and Portal Requests lists.
Scoring and Results
The Scoring section displays the client's total score, which ranges from 6 to 30. Higher scores reflect greater satisfaction with the session, while lower scores indicate less satisfaction.
In this example, the client scored 25 out of 30, suggesting moderate to high satisfaction.
The Results table displays each question from the Client Feedback Form, with the client’s responses highlighted in gray.
Downloading or Printing the Client Feedback Form
To download a PDF of the completed form, click the Download link at the bottom of the page. When the PDF is ready, click the Ready: Click to Download link.
To print the form, click the Print icon at the bottom of the page.
