The Emergency & Other Contacts Form allows you to collect important contact information from your patients, including emergency contacts and other relevant individuals. Once a patient completes the form, the submitted information is saved in their TherapyNotes chart. With practice approval, the details provided can be automatically merged into the patient's chart for streamlined record-keeping.
This form can be shared with patients through TherapyPortal, allowing them to complete and submit it electronically. Once submitted, the form will be stored in the patient’s chart for staff review.
Sharing on the Portal
You can share the Emergency and Other Contacts form on TherapyPortal for clients to complete.
To update its sharing status:
- Go to Library > Portal Forms > Emergency and Other Contacts Form.
- Choose one of the following options:
- Shareable on Demand: Available when manually shared.
- Always Available on the Portal: Clients can access the form anytime.
- Not Shareable: The form is not available on TherapyPortal.
- Click Save Changes to apply your selection.
Previewing the Emergency & Other Contacts Form
Navigate to the Library > Portal Forms > Emergency and Other Contacts Form
You can preview the form. Any time the form is in Preview, you’ll see a yellow banner stating, “This is a preview of the form, and information here will not be saved.”
Sharing on the Client Portal
- At the bottom of the screen, click Share on Portal.
- The Share on Portal dialog will appear.
- Enter the client's name and any relevant instructions for the form.
- Click Send Document Request to share the form.
