Consent for Use of Artificial Intelligence (AI) Portal Form

The Consent for Artificial Intelligence Portal Form is a customizable template available in your practice’s Library within TherapyNotes. This form allows patients to provide consent for the use of TherapyFuel AI features in TherapyNotes. Practice Administrators can modify the content of this form using the Custom Form Builder to meet their practice’s specific needs and compliance requirements.

Review and Customize the Form

Before enabling the form in TherapyPortal, you should modify it to align with your practice’s policies and other relevant state laws, licensing board requirements, and ethical codes.

1. Navigate to Library > Portal Forms > Consent for Artificial Intelligence (AI) Form.
2. Click Edit to open the form in the Custom Form Builder.
3. Modify the content to meet your practice’s and jurisdiction’s requirements.
4. Save the updated form.

Enabling the Form for Portal Sharing

Once the form is customized, you can enable patients to complete it via TherapyPortal.

1. Go to Library > Portal Forms > Consent for Artificial Intelligence Form.
2. Select Shareable on Demand, Always Available on the Portal, or Not Shareable.
3. Click Save Changes. 

Send Form to Patients

After enabling the form, you can send it to patients for electronic completion and signature.

1. Open the Patient’s Chart.
2. Click on the Portal tab.
3. Click Share Documents
4. In the dropdown, select Portal Forms and Consent for Artificial Intelligence.
5. Click Send Document Request.

Completing the Consent for AI Form as a Patient

1. The patient logs into TherapyPortal.
2. On the homepage, they will see a notification indicating they have a document for review. They can also navigate to the Documents section.
3. The patient clicks Sign, then provides either a drawn or typed signature.
4. The patient clicks Submit Completed Document to finalize the process.

Viewing and Processing Completed Forms

Once a patient submits the form, it will be available for processing and storing in their chart.

1. Navigate to the Patient’s Chart
2. Click on the Portal Tab.
3. Under Document Requests, you can find the Consent for Use of Artificial Intelligence Form with the Needs Processing status.
4. Select Needs Processing.
5. Confirm their signature.
6.  After processing, the completed form will be available to view in the patient’s Documents tab.
