The Insights tab offers a visual overview of standardized outcome measure scoring trends, making it easier for clinicians to track and interpret patient progress over time. This feature is available within each patient’s chart as a dedicated tab, providing quick access to key insights.
Accessing Outcome Measure Data
- Navigate to a patient’s chart.
- Click on the Insights tab.
- The system displays a graph plotting the patient’s outcome measure scores over time based on the administered date.
Customizing the Date Range
By default, Insights displays data for the last loaded date range. To refine the view, select a custom timeframe from the available options:
- Predefined Ranges: Current Week, Previous Week, Last 7 Days, Current month, Previous Month, Last 30 Days, Last 30 Days, Current Year, Previous Year, Last 365 Days
- Custom Date Range: Manually adjust the timeframe to focus on specific periods.
Understanding the Outcome Measure Graph
The graph provides a trend line to help visualize progress and identify patterns.
X-axis: Displays the selected date range.
Y-axis: Represents the outcome measure score range.
Data Points: Each point displayed corresponds to a recorded outcome measure score.
Hovering over a data point reveals details including:
- Score and description, if available
- Date administered
- Clinician who administered or processed it
In the Insights tab, certain outcome measure results are color-coded to offer a quick severity assessment, allowing you to interpret and track symptoms at a glance.
- Red - Highest Severity: High scores indicate severe symptoms, signaling significant distress or clinical concern.
- Yellow - Moderate Severity: Scores indicate moderate symptoms, possibly requiring monitoring or intervention.
- Blue - Mild Severity: Scores indicate mild symptoms, generally within a manageable range.
- Green - Minimal or no Severity: Scores suggest minimal or no symptoms, indicating improvement or stability.
When clicking on a specific date, you will be taken to the original outcome measure submission. Here, you can see detailed responses to questions and any sub-scores this submission may have.
Unsupported Outcome Measures in Insights
Certain outcome measures are not currently supported to be visualized in a chart on the Insights tab. If a patient has data for an unsupported outcome measure within the selected date range, it will be listed in the panel at the bottom of the tab.

- C-SSRS: Columbia-Suicide Severity Rating Scale (Screener, Recent)
- CAGE-AID: CAGE Adapted to Include Drugs
- Ch-OCl-R-P: Obsessional-Compulsive Inventory-Revised-Parent
- EAT-26: Eating Attitudes Test©
- ECR-R: Experiences in Close Relationships-Revised
- LEC-5: Life Events Checklist for DSM-5
- MDQ: Mood Disorder Questionnaire
- PHQ: Patient Health Questionnaire
- TAPS: Substance Use Screening
- VADRS: NICHQ Vanderbilt Assessment Scale
