How To: Import Client Information Into TherapyNotes


If you are leaving a different EHR and coming to TherapyNotes, welcome! If you are leaving a group practice and striking out on your own, congratulations! Whether you're transferring from another EHR or converting from paper records for the first time, the TherapyNotes team is here to help you get started quickly and easily.

We offer a service to import client information directly into your new account, free of charge. No matter how many clients you see, this can drastically reduce your startup time and help eliminate data entry errors.

Gather your client information into a spreadsheet

TherapyNotes is able to import a variety of demographic and insurance information into your new account. Refer to the table below to see what data can and cannot be imported.

If you are converting from paper records, enter as much of the importable information into an Excel document as desired, using one row for each client.

Quick Tip: If you are transferring from another EHR, you may be able to export your client information from your current EHR into a spreadsheet. We accept Excel documents (.xls or .xlsx), CSV files, and VCF files from other EHRs. Read Send your file to TherapyNotes below to learn how to begin your import.

Format your document

The most important part of preparing your data for import is consistency. While multiple formats are supported for each type of data, formatting within the file itself should be consistent.

Follow the guidelines below to ensure a quick import process:

  • Organize your file with column headers which clearly identify which type of data is in each column
  • Ensure that all dates are formatted in the same way
  • Ensure that all names are formatted in the same way
  • Ensure that all phone numbers are formatted in the same way

Below is an example of what a properly formatted document may look like.

Send your file to TherapyNotes

Please never send client information or other PHI through email - email is not a secure method of communication. Use our secure portal to send us your Excel file.

To send your Excel file or spreadsheet to TherapyNotes for import:

  • Log in to your new TherapyNotes account
  • Click the User Icon > Contact Us > Send a File to TherapyNotes Staff
  • Click the Choose File button and select your Excel file
  • Include the following information in the Description field:
    • The words Patients List
    • Your new TherapyNotes Practice Code for the account you want the information to be uploaded to
    • Your full name
    • The name of your old EHR or your old TherapyNotes Practice Code, if applicable
  • Click the Submit File button

You will receive an email confirming that we received your Excel file upload and another confirming that your import is complete. This may take between 24 and 48 hours.

Next Steps

Your clients are not assigned to clinicians when they are imported, so your client list may appear empty at first.

To see your migrated client list once the import is complete:

  • Click Patients
  • Select No Assigned Clinician from the Assigned to dropdown
  • Click the Search button

To upload additional files into TherapyNotes:

  • Click Patients > Patient Name > Notes tab
  • Click the Upload a Patient File button
  • Choose any file related to the client, such as consent forms, practice policies, or session notes
  • Click the Add Document button

    I have just signed onto TherapyNotes and have current clients for whom I have pre-existing clinical notes, treatment plans, etc. in my paper chart. I have tried uploading those documents, but cannot seem to link the upload to the scheduled service in TherapyNotes, so I can't remove the incomplete note prompt in the client's "to do"...Is there a way to tell TherapyNotes that yes, document abc.pdf replaces the "psychotherapy intake note"? This is just a temporary situation as I consolidate my pre-existing paper system to TherapyNotes

    I am trying to upload scanned documents and for all documents that over larger than 2 pages I am getting an error message that it can not be uploaded because the file is too large. Is that accurate that none of the documents uploaded can be larger than two pages?

    Kimberly, if a 2 page file is showing as too large then you should adjust your scanner settings. You likely are scanning at a very high DPI. Try using 150 dpi.

    Irene, uploaded files are not linked to appointments in TherapyNotes. You can either delete the appointment, or do a very brief note that says "See uploaded note".

    Is gender necessary? In this day and age it could be construed as insulting to only have male and female on a form so is there any way around this?

    The only purpose of the gender field is for billing.  Insurance companies currently require a binary designation (M/F) when submitting claims, which is why TherapyNotes limits the options available. However, we recognize that this binary designation does not encompass all possible gender identities.  When completing the intake note, there is an "Identification" field to document gender identity. We also encourage clinicians to use the "Patient Comments" field on the Patient Info tab to document more appropriate information. You can enter their preferred name in the "Nickname" field, and upload a picture that will appear in the chart as well. Please let us know if you have any other questions. Thank you.

    I need to download some notes for a court case. How do I do this from Therapy Notes to a flash drive to share with attorneys.

    Please follow the directions below to download notes for your patient. If you have any questions, please contact our Customer Success Team at 215-658-4550 or by email at Thank you.

    1) Click Patients > Patient Name > Documents tab
    2) Click the Download Multiple link on the bottom left.
    3) Select the checkbox for the documents you wish to download.
    4) Click the Download Selected Documents button.
    5) Follow the download instructions for your internet browser. If you are using a desktop or laptop, the batch of notes downloads as a single PDF into your hard drive's Downloads folder.

    The fact that I can send you the Excel sheet we've used in the office and not individually enter all of my clients is fantastic. Thank you for your article! That is so much easier than what I've been doing this afternoon with data entry.
