PA Sales Tax Exemption Certificate

Nonprofit, educational, and government practices in Pennsylvania must upload a signed REV-1220 Tax Exemption Certificate to avoid being charged sales tax.  

1. Locate the form in Practice Documents and Agreements. 

• Click User Icon > Settings > TherapyNotes Subscription > Sales Tax Exemption - Practice is responsible for sales tax (under Practice Documents and Agreements section)

2. Download the REV-1220 by clicking the download our Tax Exemption Certificate text link.  

Print and fill out the form, then scan it back into your computer.  

3. Upload the signed REV-1220 by clicking the Upload Signed REV-1220 button. Click Choose File to locate the scanned file.  After selecting the document click the Add Document button.
