Sales Tax Exemption Certificate

Nonprofit, educational, and government practices must upload a signed Tax Exemption Certificate to avoid being charged sales tax.  

1. Locate the form in Practice Documents and Agreements. 

• Click User Icon > Settings > TherapyNotes Subscription > Sales Tax Exemption - Practice is responsible for sales tax (under Practice Documents and Agreements section)

2. Access your state Tax Exemption Certificate by clicking the download our Tax Exemption Certificate text link.  

Print and fill out the form, then scan it back into your computer.  

3. Upload the signed certificate by clicking the Upload button. Click Choose File to locate the scanned file.  After selecting the document click the Add Document button.
